How does exploration and corridor placement work?

Quick answer

When a Character moves through an Unexplored Corridor, draw an Exploration card to place a new Room and surrounding Corridors according to the card's layout. [p.24 — Exploration Sequence]

Details

Exploration happens when a Character moves through a Corridor that connects to only one Room (an Unexplored Corridor). You then resolve the Exploration Sequence by drawing a card from the Exploration deck. [p.24 — Exploration Sequence]

The Exploration card shows which Room type to place (marked "A", "B", "C", or "?"), and you draw a random Room of that type and place it face-up at the end of the Unexplored Corridor. The card also indicates where to place new Corridors around this Room. If a Corridor has a Door slot, orient it so the slot faces the newly explored Room. [p.24 — Exploration Sequence]

Two important placement restrictions apply: do not place Corridors that would lead outside the Facility's borders, and do not place Corridors that would connect to Rooms already on the map. [p.24 — Exploration Sequence] After setting up the Room and Corridors, place any markers shown on the card (like Fire or Noise markers), move your Character to the new Room, and resolve the Entrance Effect—usually a Noise roll. [p.24-27 — Exploration Sequence and Character Movement Example]

Watch out: Exploration cards must be oriented the same way as the map (note the North icon on the card). If the Exploration deck runs empty, reshuffle all cards from the discard pile before drawing again. [p.24 — Exploration Sequence]
