Is fire damage applied once per turn or once per round?

Quick answer

Fire damage is applied once per Character's turn when they end their turn in a room with a Fire marker, but not more than once per round per Character.

Details

Fire damage to Characters occurs during the Player Phase, specifically when a Character ends their turn in a room containing a Fire marker. At that moment, the Character suffers 1 Light Wound. Importantly, even if other Characters take their turns later in the same round and the room still has a Fire marker, the affected Character does not suffer additional Fire damage that round [p.19 — FIRE MARKERS].

For Intruders, Fire damage is resolved during the Event Phase in the "Fire Damage step": each Intruder in a room with a Fire marker suffers 1 Injury [p.19 — FIRE MARKERS].

Watch out: If a Character passes (skips their actions), they still suffer Fire damage at the end of their turn, but this also grants immunity to further Fire damage for the rest of the round. Players might forget that passing doesn’t avoid the initial damage.
