Small Items with Actions can be used once per Combat (discarded after use); passive effects can be used unlimited times.
Details
Small Items come in two varieties in Combat: [p.25 — Items in Combat]
Items with Actions (shown on the back of the token): These are free Actions that can be used once during Combat. After you use an Item Action, you discard the token from your Inventory to your Pouch. You can equip it again during the next Camp Phase. [p.25 — Items in Combat]
Items with passive effects (shown in a frame on the front): These provide ongoing bonuses like additional armor or damage and are not discarded after use. You may use the benefit at any time without moving the Item token, so these can be used unlimited times throughout Combat. [p.25 — Items in Combat]
Watch out: Item Actions are free and don't count against your 2 Actions per Activation, but you can only use each Item Action once per Combat since the token gets discarded. If you want to use that Item again, you must re-equip it during Camp.
