How to deal with item after used
Quick answer
After using an Item Action, discard the Item token from your Inventory to your Pouch; you can re-equip it during the next Camp Phase.
Details
Using an Item Action is free and does not count against your two Actions per Activation. You may use it during your Activation (before or after taking an Action) or at a moment specified on the token, even outside your own Activation [p.25 — Items in Combat]. The effect of the Item Action is written on the back of the token, while the front only serves as a reminder [p.25 — Items in Combat].
Once the Item Action is resolved, discard the token from your Inventory to your Pouch. It can be equipped again during the Camp Phase, following the normal Reequip step [p.25 — Items in Combat] [p.19 — Camp Sheet & Camp Phase].
Watch out: this discard rule only applies to Items with an active effect (an Action you trigger). Items with a passive effect, shown inside a frame (such as bonus armor or bonus damage), are never discarded after benefiting from them — they remain equipped and keep providing their bonus continuously [p.25 — Items in Combat].
